LPN Profession: Nursing Specialty: Long-Term Care (LTC) Duration: 14 weeks Shift: Full Time Days and Nights Hours per Shift: 5x8 per week, Evening and Weekend/ Holiday required Experience: 1 year as LPN License: NY LPN Licensure Certifications: BLS (AHA or RQI) Must-Have: - NY LPN Licensure - BLS Certification - 1 Year Experience as LPN Description: The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit. The individual must demonstrate knowledge of the principles of growth and development across the lifespan. The LPN must possess the ability to assess data reflective of the patient's status. They must interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs. The LPN provides direct and indirect skilled nursing care for assigned residents. They follow defined policies and procedures under the supervision of a Registered Nurse. The LPN maintains a collaborative relationship with other nursing staff, physicians, and ancillary staff. They must maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs. The LPN shall have the ability to provide care to geriatric residents. Job Duties: The LPN collects data relevant to each resident's physical and psychosocial condition on an ongoing basis. They communicate their findings to the Registered Nurse and other members of the Interdisciplinary Team. The LPN contributes to the creation of a developmentally age-appropriate and culturally relevant plan of resident care. This plan should reflect nursing diagnosis and interventions to achieve desired resident outcomes. The LPN involves the resident, family, and healthcare team in this process while promoting a healthy and safe environment. They recommend revisions to the plan of care after evaluating resident outcomes. The LPN indicates the need for referrals to ensure all aspects of the resident's care have been addressed. They collaborate with the resident, family, and members of the Interdisciplinary Team within the scope of nursing practice, assuring customer satisfaction. The LPN maintains current knowledge and competency in nursing practice. They participate in the mentoring and orientation of staff. The LPN considers factors related to safety, cost, and impact in the planning and delivery of care. Under the direction of the Registered Nurse, the LPN provides leadership in the professional practice setting. They demonstrate knowledge and competency in their role. Occupational exposure to bloodborne pathogens is reasonably anticipated. Required tasks involve potential contact with blood, body fluids, or tissues.
