In this role, the individual will serve as a key connector between patients, their providers, and external healthcare organizations, ensuring smooth transitions to the right level of care, such as intensive outpatient programs, partial hospitalization programs, or specialty psychiatry.As a Care Coordinator, the individual will manage care coordination tasks in a fast-paced, detail-oriented environment. This role plays a critical part in helping patients navigate complex care decisions, while maintaining empathy and professionalism in all communications. This includes speaking directly with patients, external providers, and insurance representatives to ensure continuity of care.The individual will join a small but dedicated team of Care Coordinators within the Clinical Operations organization. The team prides itself on teamwork, operational excellence, and working together to overcome complex challenges.Required Qualifications2+ years of professional experience in customer-facing or care coordination roles1+ years of external-facing customer support experienceStrong systems fluency with the ability to quickly learn and adapt to new platformsAbility to recognize signs of patient crisis and escalate appropriatelyExperience working in a high-volume, fast-paced environmentProven ability to manage tasks within a queue- and SLA-driven workflowProficient in navigating CRMs, EMRs, and documentation toolsExperience collaborating cross-functionally with clinical, partnerships, and operations teams in a remote settingPreferred QualificationsWhile having the preferred qualifications enhances candidacy, having all of them is not mandatory.
