This position is full time, 40 hours per week. This is a professional-level position that will have the ability to work from home; however, in-state travel is required in the caseload coverage area. The overall responsibility of this position will be administering the CHOICES waiver and CTS program, and coordinating services for adults and children who have an intellectual or developmental disability. This includes, but is not limited to, coordinating services to meet medical, psychiatric, social, nutritional, educational, vocational, employment, housing, transportation, recreational, legal, and advocacy needs. Case Managers will provide support to individuals to actively engage in and direct the person-centered plan development process.\n\nThe ideal candidate lives in Chamberlain or Winner or somewhere within 30 miles of that area.\n\nEssential Duties and Functions:\nAct as part of a multidisciplinary team designed to provide services from a holistic and Values-based perspective.\nAssist in the development of recovery plans and reviews in collaboration with the program participant, clinical and medical staff and other stakeholders as determined in collaboration with program participant.\nDevelop and maintain relationships with community agencies in order to assist program participants in connecting with all available resources.\nFacilitate team process for the initial and annual development of the Individual Service Plan (ISP).\nOngoing monitoring of ISP and documenting results and intervene as necessary.\nReport and review Critical Incident Reports (CIR) per protocol.\nParticipate and monitor Human Rights Committee (HRC) and Behavioral Intervention Committee (BIC) reviews as appropriate.\nFocus on Personal Outcome measures.\nKnowledge and understanding of Charting the Life Course framework and Person Centered Thinking/Planning.\nEvaluate the needs of those on caseload in order to remain successful in the community.\nLink those on caseload to programs and supports that are needed and requested to meet their needs.\nComplete required documentation of services, including input of data, reports, and plans into Therapy.\n\nQualifications:\nBachelor’s degree and one year of experience OR equivalent combination of education and experience.\nIntermediate computer skills.\nHave the ability to establish a HIPAA-compliant home office.\nValid driver’s license.\n\nRequirements:\nLifting Requirements:\nMedium: exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects.\nPhysical Requirements:\nStand or Sit (stationary position)\nWalk\nUse hands or fingers to handle or feel (operate, activate, prepare, inspect, position)\nClimb (stairs/ladders)\nTalk/Hear (communicate, converse, convey, express/exchange information)\nSee (detect, identify, recognize, inspect, assess)\nPushing or Pulling\nRepetitive Motion\nReaching (high or low)\nKneel, Stoop, Crouch or Crawl (position self, move)\nReporting Relationships:\nDirect Supervisor: Assistant Director\n\nResources for Human Development is an equal opportunity employer.
