The Program Supervisor works within a Treatment Team to oversee all Program Activities to assure that all members are engaged and participating throughout the day.\n\nQUALIFICATIONS\n\nTo perform this job successfully, an individual must be able to perform each essential duty mentioned satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.\n\nHigh School Diploma/GED is required but prefer Bachelor’s degree or equivalent experience resulting in broad knowledge of adult day rehab programing, supervising, training, direct service delivery, budget management, leadership, communication, and time management.\nMust possess a valid and current driver’s license.\nCertified as a QBHP. Supervisory experience is preferred.\nPrefer knowledge of the mental health system, Medicaid rules, CSSP, CARF guidelines related to adult day rehab services.\nMust have proficient computer skills and basic knowledge of word and excel programs.\nHonest, dependable, self-motivated, and dedicated to Birch Tree Values and Mission.\nAbility to perform essential duties and responsibilities satisfactorily.\n\nESSENTIAL DUTIES AND RESPONSIBILITIES\n\nMust complete all required documentation by the end of their shift every day.\nMaintain current certifications as a QBHP to include Face to Face MHP Supervision every 14 days with a minimum of 12 contacts every 90 days.\nMust complete mandated Inservice training once per 12 month period. Must have a total of eight hours every twelve months beginning with the date of certification as a QBHP\nFacilitate psycho-educational groups in consultation with leadership/peers regarding topics of focus.\nMaintain an ongoing, active schedule of activities and interventions for all members.\nCollaborate with other members of treatment team and other shifts assuring communication of vital member and or other information.\nUnderstand and effectively use program materials, ensuring supplies readily available to conduct scheduled groups.\nTimely completion of work assignments, flexible hours as indicated, and log work on appropriate forms.\nOngoing Program development working closely with treatment teams, Branch Director and members coordinating a weekly program schedule and monitoring the daily delivery of services.\nSeek innovative ways to enhance the program through community engagement, service projects, and volunteerism.\nCreate a schedule of activities that promote engagement, interaction, etc. and provide to the designated supervisor one week prior to beginning the program. Keep on file a current copy of each member’s individual schedule.\nAssign member to teams for designated Rehabilitation Coaches\nAccess community resources to enhance program activities and schedule transportation.\n Successful identification of community resources and supports members seeking to use resources.\nAttends regularly scheduled meetings such as weekly supervisions, monthly observed interventions, program supervisor meetings, etc. Must have 12-13 per quarter with no more than 13 days between.\nEffectively communicates with QBHP’s on shift regarding business needs resulting in few instances of miscommunication that disrupt flow of services.\n All documentation is completed timely and efficiently with late services below 8%.\nMaintain program supplies, effectively use program materials, and ensure supplies are readily available to conduct scheduled groups.\nMaintain healthy relationships with members, always respecting appropriate boundaries.\nMust have the ability to access and transport members in all company vehicles as applicable. This will require a valid driver’s license.\nThe ability to work in a constant state of alertness and in a safe manner.\nMust be able to work shifts assigned based on the needs of the members and/or branch.
