Job Title: Physical TherapistLocation: Fort Smith, AR, 72901Duration: 03 Months with possible contract extensionShift: Mon to Fri (08:00 AM to 04:30 PM)Hours: 40 hours per weekCoverage Areas: Fort Smith and surrounding areasWill Milage be reimbursed between visits, from first patient to last patient (Y/N)? YWhat are the top responsibilities for this position? (Please be as detailed as possible):Evaluates the patient's functional status, status of all body systems as required for CMSdocumentation, and physical therapy needs. Consults with the physician in the developmentof the therapy plan of care.• Provides services within the scope of practice as defined by the state laws governing thepractice of physical therapy, in accordance with the plan of care, and in coordination withother members of the health care team.• Observes and reports the patient's response to treatment and any changes in his/hercondition to the nurse supervisor and/or the physician.• Treats patients to relieve pain, develop and restore function, and / or maintain maximumfunctioning using acceptable standards of practice and evidence based techniques.• Instructs and informs participating members of the health care team, the patient, and thefamily/caregivers regarding the plan of care, functional limitations, and progress towardgoals.• Visits patients within the entire service area of their respective agency and other nearby LHCagencies as needed.• According to Plan of Treatment, completes a progress note for each visit, and submitsprogress notes per LHC policy.• Completes all patient evaluations and develops the PT plan of care within state specificguidelines. Reports outcomes of evaluation, goals, and anticipated projected frequency ofcare to nurse supervisor within 24 hours of the evaluation.• Participates in staff conferences and committees as requested by the agency.• May supervise home health aides per LHC policy if allowed per state regulation.• Provides supervision of the PTA as per LHC policy and state regulation.• Schedule flexibility is needed in this role to ensure we provide care to the patient as needed.Requires the ability to participate in a weekend rotation and potential availability on observedholidays to ensure we are meeting the demands of our patients.• Prepares a written discharge summary and/or a written summary report on patient'scondition at least every 60 days.• Provides in-service education as requested by agency leadership.• Adheres to LHC Therapy Standards of Practice in provision of services to all patients.• Is responsible for accessing education to maintain and enhance clinical knowledge and skillsthat are up to date with the current standards of care for the profession and to maintain stateissued licensure.• All other duties as assigned.Must have qualifications/experience: Home Health and OASIS experience requiredWhat skills/attributes are nice to have, and will set a candidate apart?
