As a Hospice RN Case Manager, you'll provide skilled nursing care and compassionate support to patients and families during life's final chapter, helping ensure comfort, dignity, and quality of life. You'll have the autonomy to coordinate comprehensive care for your caseload while working collaboratively with our interdisciplinary team to address the physical, emotional, and spiritual needs of those in your care.What You'll DoProvide Expert Hospice Nursing CareConduct initial evaluations and ongoing assessments of patients' nursing needsDeliver skilled nursing services requiring specialized expertiseDemonstrate proficiency in medication administration across all routes (IM, SQ, SL, TD, PO, enteral tube, IV)Manage complex symptoms and provide comfort-focused interventionsSafely and effectively use durable medical equipment and medical devicesCoordinate Comprehensive CareAssume primary responsibility for your caseload, including assessment, planning, implementation, and evaluationDevelop and maintain individualized plans of care in collaboration with physicians and the interdisciplinary teamCoordinate services required for hospital discharges to home or facility settingsSupervise and document home health aide and LPN care deliveryFacilitate smooth transitions and ensure continuity of care across all settingsCollaborate with the Interdisciplinary TeamParticipate actively in IDG meetings, offering clinical insights and recommendationsAttend and contribute to nursing facility care conferencesCommunicate regularly with physicians, facilities, team members, and the Hospice Medical DirectorCoordinate with social workers, chaplains, aides, and other team members to address holistic patient needsSupport Patients and FamiliesBuild rapport and communicate effectively with patients, families, and caregiversAssess home safety, environment appropriateness, and support system capacityProvide education about symptom management, medications, and end-of-life careOffer appropriate support at time of death and conduct bereavement assessments when requestedRespond with sensitivity to family emotional stress and respect diverse lifestylesMaintain Excellence in Documentation and ComplianceComplete all documentation within 24 hours of visitsDocument medication regimens and update profiles according to proceduresEnsure clinical notes reflect ongoing communication and care coordinationMaintain compliance with Conditions of Participation and third-party payor requirementsIdentify and report potential payment or coverage issues promptlyWhat We're Looking ForRequired QualificationsGraduate of an accredited Diploma, Associate, or Baccalaureate School of NursingCurrent State License as a Registered NurseMinimum 1 year of nursing experience in a clinical care settingReliable transportation with valid automobile insuranceWillingness to travel extensively within service area dailyPreferred QualificationsBachelor of Science in Nursing (BSN)Experience in hospice or home health environmentEssential Skills and CompetenciesAdvanced nursing assessment and clinical judgment skillsProficiency in phlebotomy with proper implementation of universal precautionsAbility to assess patients in varied home and facility environmentsStrong time management skills with ability to handle multiple priorities and deadlinesExcellent communication and rapport-building abilitiesCultural sensitivity and respect for diverse lifestyles and belief systemsAbility to cope with family emotional stress and end-of-life situationsSelf-directed with strong organizational skillsKnowledge of hospice philosophy and palliative care principlesWhat Makes You SuccessfulYou're a skilled nurse who sees hospice care as a calling, not just a job. You'll work independently in varied environments and must be comfortable adapting to different settings and situations. The role has a high degree of schedule variability due to patient needs, admissions, and end-of-life situations. Flexibility and availability (including potential on-call duties) are essential.
