We are looking for someone who is passionate about supporting older adults and helping participants maintain their independence and quality of life.\nSchedule:\nMonday – Friday\n8:00 AM – 2:00 PM\nPart-Time\nPreferred Qualifications:\nBilingual in Spanish and English (strong verbal and written communication skills preferred).\nResponsibilities:\nConduct psychosocial assessments and ongoing participant evaluations.\nMonitor participants' emotional, social, and behavioral well-being.\nComplete home assessments as needed to evaluate living conditions and identify participant needs.\nDevelop individualized care plans and provide ongoing case management.\nFacilitate problem-solving and support discussion groups.\nAssist participants and families with accessing community resources, benefits, and supportive services.\nProvide counseling, education, and emotional support to participants and caregivers.\nServe as a liaison between participants, families, caregivers, healthcare providers, and community agencies.\nParticipate in Multidisciplinary Team (MDT) meetings and contribute to participant care planning.\nMaintain timely, accurate, and confidential documentation of all assessments, interventions, referrals, and participant interactions.\nCollaborate with nursing and rehabilitation staff to ensure comprehensive participant care.\nMinimum Qualifications (California CBAS Requirements):\nApplicants must meet one of the following qualifications:\nBachelor's degree in Social Work from an accredited school and at least two (2) years of experience providing social services in aging, healthcare, or long-term care; or\nMaster's degree in Social Work from an accredited school of social work; or\nMaster's degree in Psychology, Gerontology, or Counseling from an accredited institution and at least one (1) year of experience providing social services in aging, healthcare, or long-term care.\nIf you are committed to making a meaningful difference in the lives of older adults and enjoy working in a collaborative healthcare environment, we encourage you to apply.
