Community Networking Support

On a daily basis, assists Program Manager in implementing individual programs in community settings for individuals by performing the following duties. Assist individuals in achieving their goals. Learns the personalities, habits and routines of the individuals and works with them accordingly. Provides encouragement and support to the individuals so they can achieve their desired objectives. Teaches, supports, and models appropriate social skills when working with the individuals. Encourages and empowers the individuals to express their wants, needs and individual preferences. Actively works with staff and program participants to “problem-solve” issues or concerns which may arise Provides active Individual Support Plan (ISP) team participation, including development of ISP objective and attending ISP meetings as needed. Documents progress on ISP objectives daily in a thorough and accurate and person centered manner. Provides opportunities for community recreational activities. Provides transportation as needed. May be required to work in the community as needed. Maintains good communication with supervisor. Keeps him/her informed of developments, concerns, and any other pertinent issues. Is responsible for knowledge and implementation of agency and departmental policies and procedures. Projects a positive image of Moniteau County SB 40 Board and individuals to the local community Assures the confidentiality of information regarding the individuals. Reports to the Supervisor(s) any medical issues of the individuals served. Completes General Event Reports accurately and completely within the timeframe established by policy.
