Licensed Practical Nurse – Home Health Schedule: Days Assignment Details: Guaranteed Hours: 36 Contract Length: 13 weeks Requirements: Must be a Licensed Practical Nurse with an unencumbered compact license. Graduate from an approved school of practical nursing. 2 years of experience in a home health setting. Roles & Responsibilities: Provide nursing services to individuals and families in their residence under the supervision of a registered nurse. Perform prescribed individualized patient care to patients as delegated. Assist the registered nurse in the preparation, evaluation, and modification of the patient's plan of care as appropriate. Observe, document, and report to the registered nurse the patient's physical and emotional needs and reactions to care.
