CNA - Certified Nursing Aide for longterm skilled care facility 12 hour day shift

Duties:
- Provide direct patient care, including assisting with activities of daily living and ensuring patients' physical and emotional needs are met.
- Assist with food intake and hydration, adhering to established procedures and protocols.
- Monitor patients' vital signs and report any changes or concerns to the healthcare team.
- Assist with showers, personal hygeine, toileting, and other treatments as needed.
- Collaborate with nurses and other healthcare professionals to develop and implement individualized care plans.
- Maintain accurate and detailed records of patient care and treatment in easy to use eMar.
- Participate in patient education and discharge planning to ensure a smooth transition to home care.
- Perform other duties as required to support the healthcare team and ensure high-quality patient care.
Requirements:
- High school diploma or equivalent required.
- Completion of a certified nursing assistant program.
- Current certification as a Nursing Assistant (CNA).
- Basic understanding of medical terminology and physiology knowledge.
- Familiarity with electronic medical records (EMRs) and healthcare systems.
- Ability to work effectively in a fast-paced environment and prioritize tasks as needed.
- Strong communication and interpersonal skills.
- Ability to work flexible shifts, including weekends and holidays.
- Commitment to maintaining confidentiality and adhering to HIPAA regulations.
