Social Worker

Position Summary Join a compassionate, collaborative health care team where your clinical expertise makes a measurable difference in patients' lives. As a key provider of psychosocial care, you will conduct thoughtful, comprehensive assessments; partner with the medical team to create and implement person-centered plans; and connect patients and families with community supports that reduce stress and promote recovery. You will deliver services consistent with organizational policies and applicable regulations while helping to shape positive patient experiences and outcomes.Key AccountabilitiesPerform comprehensive psychosocial assessments to identify and address patients' and families' social, emotional, and practical needs that affect treatment and recoveryConduct suicide risk assessments and recommend appropriate safety measures to the interdisciplinary teamCoordinate referrals and collaborate with multidisciplinary colleagues to link patients and families to community and hospital resources that support continuity of careProvide targeted therapeutic interventions, including crisis intervention, grief and supportive counseling, strength-based problem solving, psychoeducation, and strategies to enhance healthy copingWork closely with medical staff to develop patient-centered care plans and identify psychosocial barriers that may delay or prevent dischargeMaintain timely, accurate electronic documentation and data collection in the medical recordCommunicate consistently with team members about patient needs and interventions; actively participate in team meetings, huddles, and roundsQualificationsMaster's Degree in Social WorkCompletion of an approved internship program (completion of JHACH intern program or a comparable external clinical internship)Strong listening skills and excellent verbal and written communication abilitiesProficiency documenting in electronic medical records and attention to accurate recordkeepingWhy You'll Love Working HereBe part of a multidisciplinary team that values collaboration, respect, and patient-centered careUse your clinical skills to make a tangible impact on patients' recovery and quality of lifeAccess ongoing professional development opportunities and a supportive environment that fosters growth and excellenceIf you're passionate about delivering high-quality psychosocial care and thrive in a dynamic, team-oriented setting, we'd love to hear from you.Salary Range: Minimum 27.85/hour - Maximum 44.57/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
