NO WEEKENDS!!","description":"Join our dynamic Day Program as a Qualified Developmental Disabilities Professional (QDDP) — a role dedicated to empowering individuals with developmental disabilities through compassionate, person-centered support. This position offers the exciting opportunity to work during daytime hours with no on-call responsibilities and absolutely no weekends! You’ll be a vital part of a team committed to enhancing quality of life, promoting independence, and fostering community engagement. If you’re passionate about making a difference in the lives of individuals with disabilities this is the perfect role for you!\nDuties\nDevelop, implement, and monitor individualized care plans tailored to each individuals' needs.\nAssist individuals with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts or other assistive devices, and assisting with meal prep including G-Tube feedings.\nAdminister medications accurately following medical orders while maintaining compliance with HIPAA regulations and documentation standards\nConduct regular assessments of individuals’ health status by monitoring vital signs and observing behavioral changes while documenting and charting the assessments on a regular basis.\nCollaborate with interdisciplinary teams including nurses, social workers, case managers, and families to coordinate comprehensive care strategies\nManage behavioral challenges through behavior management techniques and conflict resolution skills to create and keep a safe environment\nMaintain detailed documentation within Electronic Medical Record (EMR) systems, including care plans, incident reports, and progress notes\nSupport individuals during community outings or transportation by driving safely and adhering to all safety protocols\nMaintain accurate, current, and timely daily and monthly documentation.\nRequirements\nProven experience working with individuals with disabilities\nKnowledge of medical terminology, medication administration procedures, and hospice or memory care practices\nFamiliarity with EMR systems for accurate documentation and case management\nAbility to perform heavy lifting safely when assisting individuals with mobility or transfers using Hoyer lifts or similar equipment\nValid driver’s license with a clean driving record for transportation responsibilities\nCertification in first aid and CPR required\nStrong interpersonal skills to effectively communicate with residents, families, and team members while maintaining confidentiality per HIPAA guidelines\nFlexibility to adapt quickly to changing resident needs and work collaboratively within a team environment\nPrevious QDDP experience in a Day Program preferred\nBachelor’s Degree or Equivalent in experience\nJoin us in creating a nurturing community where every individual receives respectful, person-centered care.
