Manager - Care Management

Overview: The Manager of Care Management, under the guidance of the Director of Care Management, provides leadership, coaching and mentoring of the Care Management department professional and administrative staff in order to support and ensure standards of practice for service delivery and organizational performance strategies are consistent with PHC's Mission, Vision and Values.
