As a Home Health Clinical Manager, you'll oversee all aspects of patient care service - from admission through discharge - while developing and mentoring clinical staff, ensuring regulatory compliance, and building relationships with referral sources that drive quality patient outcomes. This leadership role is perfect for an experienced RN or PT who wants to leverage clinical expertise in a management capacity. You'll balance strategic oversight with hands-on clinical guidance, ensuring that every patient receives coordinated, high-quality care while supporting your team's professional growth and success.What You'll DoProvide Clinical Leadership and OversightOversee all patient care services for your assigned team, ensuring quality and continuityProvide expert clinical guidance on complex cases and challenging situationsEnsure coordination of care through oversight of professional practice standardsConduct supervisory visits and consultations to advance care knowledgeMaintain availability during operating hours to support patient care needs and staff questionsLead and Develop Your TeamSupervise RNs, PTs, OTs, MSWs, and home health aides, providing clinical direction and supportConduct performance appraisals and provide coaching based on needs assessmentsParticipate in hiring and orientation of new clinical personnelMeet regularly with field staff to provide guidance on clinical and operational issuesIdentify training needs and coordinate in-services and educational opportunitiesAddress performance issues and provide corrective action when neededEnsure Quality and ComplianceEnsure compliance with state licensure, Medicare certification, and accreditation standardsMonitor OASIS assessments and care plan development for accuracy and completenessImplement QAPI recommendations to improve patient care and operationsCoordinate utilization review activities to optimize care deliveryStay current with Medicare, Medicaid, and third-party payor regulationsDevelop and implement policies and procedures that support regulatory complianceCoordinate Patient Care and ReferralsOversee patient and personnel assignments to ensure appropriate care deliveryEnsure plans of care are developed, implemented, and updated based on patient needsCollaborate with physicians, hospitals, SNFs, and case managers on care coordinationConsult with QA Specialists on complex care plans and documentationEnsure staff availability and competency to respond to referrals promptlyCoordinate services with providers, vendors, and facilitiesManage Operations and ResourcesMonitor employee overtime, absenteeism, and scheduling efficiencyProvide input to administrative and EMR systems related to patient careCoordinate weekend on-call services provided by staffAssist business development team with coordinated customer effortsReport accidents, incidents, or unusual occurrences immediatelyMay serve as backup for other clinical managers as neededBuild Strategic RelationshipsMaintain professional relationships with referral sources including hospitals and physiciansRepresent the organization in marketing and public relations activitiesEnsure customer expectations are met through responsive, quality care deliverySupport growth initiatives through clinical excellence and relationship buildingWhat We're Looking ForRequired QualificationsGraduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing OR Licensed Physical TherapistCurrent State License (RN or PT)Reliable transportation with valid automobile insuranceAvailability during all operating hours with flexibility for schedule disruptionsPreferred QualificationsBachelor of Science in Nursing (BSN) for RN candidates2-3 years of home health experiencePrevious supervisory or management experienceEssential Skills and CompetenciesExcellent observation, verbal, and written communication skillsStrong clinical judgment and ability to provide expert guidance on complex casesKnowledge of business management and healthcare operationsUnderstanding of governmental regulations, Medicare/Medicaid requirements, and CHAP standardsProficiency with home health EMR systemsAbility to assess and respond to varied patient, family, and staff needsStrong leadership and personnel management capabilitiesAbility to handle multiple priorities, deadlines, and frequent disruptionsCultural sensitivity and tolerance for diverse lifestylesConflict resolution and problem-solving skillsWhat Makes You SuccessfulYou're an experienced clinician who excels at seeing the big picture while managing important details. The role requires high flexibility - you must be available during all operating hours and responsive to urgent clinical and operational needs. Frequent disruptions to routine are expected as you respond to patient emergencies, staff questions, referral source requests, and operational challenges.
