Wherever you work among our many locations around New York City, ADAPT offers paid training, competitive benefits, and we foster a team culture of learning, support, collaboration and career growth.\nSUMMARY\nUnder supervision of the Program Director, the Case Manager is responsible for developing, implementing, and monitoring Life Plans for the people we support in one of our residential settings. This role ensures that services align with regulatory requirements, agency standards, and the personal goals of each individual. The Case Manager provides advocacy, coordination of services, and family engagement, while promoting dignity, independence, and quality of life for the people we support.\nESSENTIAL DUTIES AND RESPONSIBILITIES\nThe duties and responsibilities of the Case Manager will include, but are not limited to the following:\nProvides social services to each person we support working toward maximizing their social functioning, enhancing coping capacity of their family, asserting and safeguarding their human and civil rights, and fostering their human dignity and personal worth.\nDevelops, monitors, and updates Individualized Service Plans in accordance with regulatory and agency policies.\nParticipates in and chairs interdisciplinary team meetings; documents, interprets, and follows up on all recommendations.\nCollaborates with residential staff, clinical staff, and service providers to ensure coordinated support.\nIdentifies individual aptitudes, interests, and preferences and develops goals and activities that reflect person-centered planning.\nDocuments functional assessments and reviews plans of care as needed.\nProvides supervision, training, and guidance to Residence Program Specialists on Life Plan implementation, data collection, and best practices.\nMaintains regular and meaningful contact with families, guardians, and community stakeholders.\nAdvocates for the rights of the people we support and ensures benefits and entitlements are applied for in a timely manner.\nCoordinates admissions, including referral materials, family contacts, and arrangements for day programming.\nServes as liaison to day programs, clinics, service coordination, and other external partners.\nProvides support in activities of daily living and community outings as needed.\nEnsures adaptive equipment is available, maintained, and used appropriately.\nMaintains accurate, timely documentation, including monthly case notes, progress updates, and family contacts.\nAssists with discharge planning and systematic follow-up to ensure smooth transitions to community-based services.\nHandles sensitive and confidential information with the highest level of discretion.\nKeeps abreast of changes in the field and applies updated knowledge to practice.\nPerforms other related duties as assigned.\nQUALIFICATIONS\nTo perform this job successfully, an incumbent must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.\nBachelor’s Degree in Human Services or related field required.\nMinimum of one (1) year of experience working with individuals with developmental disabilities required.\nAbility to read, write, speak, and understand English.\nStrong interpersonal and communication skills with the ability to interact effectively with people supported, families, staff, and outside agencies.\nExcellent organizational and time management skills with the ability to handle multiple priorities.\nProficiency in Microsoft Office and electronic record systems.\nAbility to maintain confidentiality and exercise discretion in handling sensitive information.\nAbility to work independently, as part of a team, and with flexibility in scheduling.\nAbility to work flexible hours.\nCOMPENSATION: $28.34 - $29.43/hour (after probation) + Industry-Leading Benefits for all full-time employees.\nAt ADAPT, we value diversity, equity, inclusion, accessibility, and belonging.
