RN Clinical Manager – Home HealthThe RN Clinical Manager is responsible for leading clinical operations within a home health setting, ensuring high-quality, compliant, and patient-centered care. This role oversees clinical staff, coordinates care delivery, and drives performance-improvement initiatives to enhance patient outcomes and operational efficiency.You'll serve as a key leader, supporting interdisciplinary collaboration while ensuring adherence to regulatory standards and agency policies.Key ResponsibilitiesLead and oversee clinical operations to ensure safe, high-quality patient care in compliance with state and federal Conditions of Participation (CoPs)Supervise and support clinical staff, ensuring appropriate care planning, reassessment, and continuity of servicesCoordinate and collaborate with interdisciplinary teams to optimize patient outcomesMonitor and manage the QAPI program, implementing corrective actions as neededEnsure compliance with Medicare, Medicaid, and other third-party payer requirementsIdentify operational or clinical issues and communicate findings to leadership with actionable recommendationsPromote infection control practices and adherence to safety standardsSupport staff development, training, and ongoing educationParticipate in interdisciplinary group (IDG) meetings and performance improvement initiativesFoster a culture of accountability, communication, and clinical excellenceQualificationsCurrent Registered Nurse (RN) license (required)Minimum 2+ years of home health management experienceStrong knowledge of home health regulations and compliance standardsExperience with HomeCare HomeBase (HCHB) preferredCurrent CPR certificationValid driver's license and auto insuranceBenefits & PerksMedical, Dental, and Vision Insurance401(k) with immediate full vestingCompany matches 100% of the first 3% contributedHealth Savings Account (HSA)Life InsuranceShort- and Long-Term DisabilityPaid Time Off (PTO) + Paid Holidays
