Healthy Start Case Manager - FT8

Description Purpose: Provide comprehensive case management services (including family support, health education, and child development services) to pregnant women, interconceptional women, and/or infants/toddlers/children through five years of age and their families as part of the Healthy Start & Empowerment Projects. Provide information, health education, support, resources and referrals to participants to address family needs; parenting and self-advocacy skills; optimize perinatal and child health; promote child growth and development; and help families to progress toward identified goals. Responsibilities:Work with a caseload of pregnant and interconceptional women, and infants/toddlers/children through five years of age and their families as part of a project effort to reduce the infant mortality and low birth weight rates as well as ethnic/racial disparities in perinatal outcomes in the Healthy Start/Empowerment Project AreasPlan, prepare and conduct regular home visits with participants/families engaged in the Healthy Start/Empowerment Projects. Maintain an average of 3 completed visits or attempted visits/completed visits per dayFacilitate the identification of family goals and strategies to achieve identified goals as desired by families with appropriate referrals and connections to community resourcesProvide appropriate referrals with each family, assisting with access to community resources and collaborating with community agencies as appropriateRespond appropriately using crisis intervention and problem solving techniques with families as neededConduct child development, depression, and health screenings; monitors and supports health promotionUtilize prenatal, child development and parent education curricula with families during home visits at a minimum of 75% of face to face visitsAssist participants in establishing and maintaining medical/dental homes to obtain perinatal care; and comprehensive health care particularly for infants and/or toddlersProvide information, advocacy, support and access to resources for breastfeeding, family planning and interconceptional careMaintain accurate documentation of records and adhere to data entry guidelines, including participant evaluationsImplement and maintain Healthy Start and Empowerment Projects' Best Practices and StandardsAttend monthly case management task force meetingsParticipate in team meetings, supervision, case consultation, staffing, and training as required by Healthy Start and Empowerment ProjectsManage time, organization, and scheduling to meet client needs as well as other job duties and responsibilitiesUse a family-centered; culturally sensitive approach to provide support and assistance to familiesUtilize language interpretation/translation services and obtain pertinent information according to the culture and primary language of each pregnant or interconceptional woman, infant or toddler and their family as appropriateConnect families to a wide-range of community resources and services responsive to the needs of families with pregnant women and/or young childrenEngage in on-going personal professional development and trainingOther duties as assigned by supervisor Qualifications:High School Diploma/GED with 10 years' experience with young children, parents and/or families or an associate degree and at least 5 years full time work experience in Nursing; Sociology; Social Work; Child and Family Studies; Psychology or other human service field required.Bachelor's degree in nursing, Sociology, Social Work, Child and Family Studies or Psychology highly preferredExperience with home visiting preferredFluent written and verbal bi-lingual communication skills in English and Spanish, Vietnamese, Arabic, or Burmese preferredAbility to accept and work with diverse populationsAbility to complete an assessment, care plan and tracking formsGood written and oral communication skillsAbility to physically carry out various aspects of providing services in the participant/client's home in a variety of environments over which there is very limited controlGood working knowledge of community resources and accessing these resourcesComplies with accepted professional standards and practiceAbility to work effectively with a multi-disciplinary teamAbility to work with the management information system and complete data entryMandatory Reporter of Child and Dependent Adult abuse training certification (within 30 days of hire)Must have valid driver's license, auto liability insurance and reliable transportation if driving is requiredSuccessful completion of pre-employment physical, drug screen, 2-step TB test and background check Working Conditions:Physical requirements: Light work, sitting most of the time, exerting up to 20 pounds of force frequently, and/or a small amount of force constantly to move objects.Physical activities: Climbing, Balancing, Stooping, Kneeling, Crouching, Crawling, Reaching, Standing , Walking, Pushing, Pulling, Lifting, Talking, Fingering (picking, pinching, typing, etc.), Hearing, Grasping (applying pressure with fingers and palm), Repetitive motions, Feeling (perceiving size, shape, texture, temperature by touching), Visual acuity (field of vision for analyzing data, viewing computer, using measurement devices, operating machines, determining accuracy or neatness, and/or making general observations)Environment: Exposure to varied temperatures, communicable diseases, dust, and stairsTime: Ability to be flexible with a varying schedule and programmatic requirementsTravel: Required between locations These characteristics are representative of those an employee encounters while performing the essential functions of this job.
