In this vital role, you will provide comprehensive nursing care to patients transitioning from intensive care to less acute settings, ensuring optimal recovery and safety. The ideal candidate will possess strong clinical expertise, excellent patient assessment skills, and the ability to adapt to a fast-paced healthcare environment. This position offers an opportunity to contribute meaningfully to patient outcomes through compassionate care and clinical excellence.\nKey Responsibilities\n- Monitor and manage patients in the PCU.\n- Administer medications and treatments.\n- Operate monitoring equipment.\n- Document care in EHR systems like Epic.\n- Collaborate with healthcare teams.\n- Educate patients on recovery plans.\nDuties\nPerform detailed patient assessments, including vital signs, physical examinations, and physiological monitoring, to determine individual care needs.\nAdminister medications and treatments accurately, including IV infusions, injections, and specialized therapies such as botulinum toxin treatments.\nMonitor patients’ responses to interventions, manage acute pain, and adjust care plans accordingly.\nUtilize electronic health record (EHR) systems such as Epic or Cerner for documentation of patient care activities, medical records management, and compliance with HIPAA regulations.\nCollaborate with multidisciplinary teams—including physicians specializing in cardiology, ophthalmology, infectious disease, and behavioral health—to develop and implement individualized care plans.\nAssist with complex procedures such as catheterizations, tube feedings, ventilator management, and sterile techniques like suturing.\nProvide education to patients and families regarding treatment plans, medication administration, discharge instructions, and health coaching.\nSupport triage processes in emergency or urgent care settings by prioritizing patient needs based on acuity levels.\nEnsure infection control protocols are strictly followed to maintain aseptic technique during all procedures.\nParticipate in discharge planning and case management activities to facilitate smooth transitions of care across settings such as home health or long-term care facilities.\nSkills\nProven experience working in critical care environments including ICU or Level I/II trauma centers; familiarity with trauma medicine is highly desirable.\nProficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth; experience with medical documentation standards including ICD-10/ICD-9 coding is essential.\nStrong knowledge of physiology, anatomy, pharmacology related to acute and chronic conditions.\nSkilled in performing advanced nursing procedures such as phlebotomy, venipuncture, ostomy care, NG tube management, and laboratory specimen collection.\nExperience managing diverse patient populations including geriatrics with memory or dementia care needs; pediatric or neonatal resuscitation experience is a plus.\nAbility to handle complex cases involving infectious diseases, hospice & palliative medicine considerations, or occupational health scenarios.\nExcellent communication skills for effective collaboration within multidisciplinary teams and with patients' families.\nKnowledge of Workers' Compensation law related to occupational injuries is beneficial.\nDemonstrated ability to adhere strictly to HIPAA regulations while managing sensitive medical information.\nThis role offers a dynamic environment where your expertise in nursing practice can significantly impact patient recovery trajectories.
