Our team provides expert guidance and hands-on assistance to help clients navigate the challenges of aging, ensuring safety, independence, and quality of life.Key ResponsibilitiesConduct comprehensive assessments and create tailored care plansOffer supportive counseling and crisis intervention as neededCoordinate medical care, resources, and community servicesFacilitate family meetings and provide education on dementia/Alzheimer's careProvide follow-up visits, progress reports, and ongoing evaluationsMarket and build relationships with referral sourcesMaintain accurate, timely documentation and uphold professional standardsPhysical RequirementsThis role involves direct interaction with clients and occasional physical tasks to support their needs.
