Clinical Pharmacist, Pharmacy OutreachThe Clinical Pharmacist, Pharmacy Outreach is responsible for optimizing patient outcomes through telephonic interventions. This role focuses on the tactical execution of medication management services, including polypharmacy, adherence, and pharmacotherapy optimization.Candidates should be comfortable working remotely from anywhere within the US, across all US time zones.Please note: Valid pharmacist license in at least 2 of the following markets and a willingness to acquire more states as needed: AL, CA, DE, KY, LA, MS, NC, OH, PA, UT, VA, or WVPrimary Duties:Clinical Execution & Patient CareCounsel patients via telephonic outreach on medication adherence, address any medication related problems such as adherence barriers, polypharmacy, or high risk medication use, and make appropriate evidence-based recommendations to patients and Practice Care TeamsPerform focused clinical chart reviews as needed to inform patient interventionsObtain and document patient commitment to medication adherence and address barriers in real timeEscalate complex clinical cases to prescribers or interdisciplinary teams as appropriateMeet and sustain performance goals (e.g., STARs, HEDIS)Clinical Innovation and Resource OptimizationCollaborate with Physicians, clinical and administrative staff to optimize the Medication Management workflow and patient experienceProactively identify gaps in medication management through patient interactions and translate patient insights into targeted recommendations that drive improvements in practice operations and clinical workflowsParticipate in the design, implementation, and evaluation of pilot programs or process changes ("test and learn") within the outreach modelServe as a clinical subject matter expert, leveraging clinical pharmacy expertise to support organizational objectives and adapt to evolving healthcare needsDevelop and maintain standardized resources, educational tools, and clinical protocols to support improved medication outcomes and performance on quality measures (e.g., STARS, HEDIS) for practices, clinicians, and patientsOther duties as assignedMinimum Qualifications:Doctor of Pharmacy (PharmD)Active, unrestricted pharmacist license in the state of practice; Valid multi-state licensure in at least two of the following states: AL, CA, DE, KY, LA, MS, NC, OH, PA, UT, VA, WVWillingness to obtain and maintain additional licenses within 6 months of hireMinimum of 3 years experience in value-based care, ambulatory care, or community pharmacy settingsIn depth knowledge of pharmacotherapy and pharmacy practiceStrong verbal and written communication skills; Ability to translate complex clinical data into clear, understandable language for patients and care teamsProficiency in telephony platforms and various Electronic Health Record (EHR) systemsAbility to work in a fast paced environment and adapt quickly to changeAbility to work between 8 AM - 7 PM EST as dictated by business needsPreferred KSA's:Familiarity with Medicare, HEDIS, and STAR ratingsPhysical Requirements:Prolonged periods of sitting at a desk and working on a computer$96,000 - $135,000 a year Salary Range: $96,000 to $135,000 a year + bonus + equity Compensation for the role will depend on a number of factors, including a candidate's qualifications, skills, competencies and experience.Aledade, a public benefit corporation, exists to empower the most transformational part of our health care landscape - independent primary care.
