We are so glad you are interested in joining Sutter Health!OrganizationSCAH-Sutter Care at Home - ValleyPosition OverviewThe ideal candidate will provide expert nutritional consultation services to patients, referral sources, and staff, ensuring enteral and Total Parenteral Nutrition (TPN) needs are assessed and managed according to established protocols, physician or case manager requests, and nutritional standards of care. Dietary plans will be tailored to consider the emotional, social, environmental, and economic context of each patient and their family.This role requires a strong understanding of Medicare and Medi-Cal TPN/enteral qualifications and coverage, as well as experience in home infusion, home health, or long-term nutritional support. The candidate should be patient-centered, an effective communicator both verbally and in writing, and able to work independently with minimal guidance. Proficiency with technology, including Outlook, Teams, MS Office, and pharmacy databases, is essential.
