The Support Worker provides direct support, skill development, and community resource linkage to persons served with psychiatric and/or substance use disabilities. Services are designed to promote independence and support individuals in living successfully in the community. This role is primarily field-based (70-80%), involving regular meetings with persons served to implement service plans and assess ongoing needs.
