Speech Therapist for geriatric and pediatric patients

The Lakes Home Care - The Lakes Home Care is currently seeking a dedicated and passionate Speech Therapist (Speech-Language Pathologist) to join our growing team. The ideal candidate will assess, diagnose, and treat speech, language, voice, cognitive-communication, and swallowing disorders across various age groups, including neonatal, pediatric, adolescent, adult, and geriatric populations.Responsibilities: Conduct comprehensive evaluations and assessments of patients with speech, language, cognitive, and swallowing disordersDevelop individualized treatment plans based on patient evaluations and diagnosisProvide direct therapeutic interventions using evidence-based practicesMonitor and document patient progress, adjusting treatment plans as neededEducate patients and families on communication strategies and home programsCollaborate with physicians, occupational therapists, physical therapists, and other healthcare professionals to ensure holistic careSupervise and support Speech Therapist Assistants as required by state guidelinesImplement and train on augmentative and alternative communication (AAC) systemsMaintain accurate, timely, and compliant documentation in accordance with professional and regulatory standardsParticipate in team meetings and contribute to interdisciplinary care planningSkills and Experience: Experience working with pediatric, adult, and geriatric populations in various settings (home health, schools, hospitals, etc.)Strong understanding of speech-language development, disorders, and intervention techniquesKnowledge of IEPs, IFSPs, and documentation procedures in educational settings (if applicable)Familiarity with AAC devices and strategiesAbility to adapt treatment to critical care and medically complex environmentsExcellent communication, collaboration, and case management skillsCompassionate and patient-centered approach to careExperience with standardized assessments and outcome measurement toolsRequirements: Master's Degree in Speech-Language Pathology from an accredited programCurrent and valid State Licensure or Certification in Speech-Language PathologyCertificate of Clinical Competence (CCC-SLP) from ASHA (or CFY candidates will be considered)CPR certification (if required by state regulations)Ability to pass background check and meet health screening requirementsCommitment to continuing education and maintaining licensureThe Lakes Benefits: Competitive salaryFlexible work hours where you create your own schedule
