A company is looking for an In Reach Coach to support individuals with serious mental health needs. Key Responsibilities Engage members to provide education about permanent supportive housing and community resources Collaborate with staff to facilitate transition planning for members seeking permanent supportive housing Utilize person-centered planning methods to address individual needs and provide education on rights and responsibilities Required Qualifications, Training, and Education High School Diploma or GED required Must have at least one year of lived experience with a behavioral health condition and/or substance use Must be fully vaccinated against Covid-19 and other specified immunizations Must be a North Carolina Certified Peer Support Specialist or obtain certification within six months of employment Must have a valid driver's license and reside within 75 miles of Wayne County, North Carolina
