Job Summary:Tryfacta is seeking a Nurse Practitioner for our client in Rochester NY 14620. This is a temporary contract assignment. Position Title: Nurse Practitioner Location: Rochester NY 14620 Duration: Up to 2 years Work Schedule: M-F 8:00 AM-4:30 PMResponsibilities for this position include, but are not limited to:Developing a health profile by taking health and developmental history, analyzing clinical laboratory data and other diagnostic tests, pertinent psychosocial data, and health assessment. Reviewing presenting problem, date of onset and duration.Performing complete or focused physical examination to include blood pressure, weight and height.Ordering routine laboratory tests such as CBC, blood chemistry, urinalysis, serology, as indicated.Recording findings of health exam on appropriate forms.Assessing data, discriminating between normal and abnormal findings. Identifying abnormalities that require further study.Assisting in the development of the medical section of the treatment care plan as a member of the interdisciplinary treatment team. Evaluating the treatment plan for effectiveness and instituting necessary adjustments or corrections.Making appropriate referrals to other community health care providers, specialists for consultation, tests, treatments.Collaborating with other health disciplines to ensure continuity of care.Counseling clients and family relatives as to the nature and course of illness, medication regimen, test, or treatment procedures, etc. To be considered for this position, you should have: [Skills, Education, or Experience]:Nurse Practitioner license for the state of NY.
