In this position, you will deliver specialized occupational therapy services to adult and elderly patients, with an emphasis on enhancing safety, independence, and overall quality of life in their home settings.Key ResponsibilitiesConduct thorough assessments to evaluate patient needs.Develop customized treatment plans tailored to individual patient goals.Implement evidence-based interventions to assist patients in performing daily activities.Help patients manage functional limitations and navigate their home environments safely.Collaborate with a multidisciplinary team, including physicians, nurses, physical therapists, speech therapists, social workers, and caregivers, to ensure comprehensive care.Educate patients and their families on adaptive strategies, home exercise routines, energy conservation techniques, fall prevention, and the effective use of assistive devices and medical equipment.Regularly reassess patient progress and modify care plans based on clinical evaluations.Accurately document all services provided in compliance with organizational policies, payer requirements, and regulatory standards.Working EnvironmentIn a culture that values compassion, integrity, and excellence, you will maintain the highest standards of patient safety and clinical quality.
