Field Nurse Practitioner
The Nurse Practitioner provides primary and preventive medical care in diverse settings such asin-home, community, nursing facilities, and pop-up clinics, focusing on improving member health outcomes.Key duties include conducting comprehensive evaluations, managing chronic and acute conditions,ordering diagnostics, developing care plans, and coordinating post-discharge care to reduce hospital readmissions.They perform face-to-face visits, participate in community outreach, and may prescribe medications and perform procedures.Travel within the assigned region is required, sometimes exceeding 120 miles daily, with occasional overnight stays.The role involves collaborating with physicians, maintaining documentation in EMR systems, and working independently with minimal supervision.Qualifications include active RN license, national certification, DEA registration, BLS, and at least 1 year of experience, with preferred experience in community or home health settings.
