Develops and monitors treatment plan collaboratively withconsumers by defining goals, specific outcomes and time-frames fortreatment. Monitors consumer\'s behavior and makes detailed reportsfor medical necessity in regular comprehensive evaluations. Intervenes therapeutically to change attitudes, beliefs andbehaviors of consumers to improve individual and family level offunctioning. Provides crisis intervention to stabilize consumer\'s condition byutilizing short-term crisis intervention techniques and/or referralto other support services. Maintains networking and referral contacts with other human serviceproviders bothinternally and externally. Carries a caseload of clients enrolled in Community Support Teamservices and as assigned by direct supervisor. Completes required monthly direct service hours to consumers asdefined on Direct Service Requirement sheet Facilitates comprehensive consumer care by providing case managementservices and therapeutic intervention in community-based settings. Facilitates recreation/socialization and cultural activities forconsumers which provide enjoyable outings that prepare and encouragethem to engage in activities independent of the agency. Effectively teaches life-management skills to consumers and assistwith the development of independent living activities, such asgrocery shopping, attending appointments, social skills training,money management, and coping skills. Provides and arranges transportation services to consumers whichthey translate to their natural environment. Demonstrates active involvement in supervision, team meetings, andclinical staffing. Collaborate withinternaland external members of theinterdisciplinary team to coordinate services and ensure the clientsneeds are being met.
