You will ensure high-quality service delivery, regulatory compliance, and the development of person-centered individual service plans while fostering strong relationships with vendors, staff, and stakeholders.\n\nKey Responsibilities\n\nTraining, Supervision & Mentoring\n\nOversee scheduling, training, and compliance for program staff.\nDevelop and implement task lists and activity schedules for Assistant Program Coordinators.\nProvide coaching, mentoring, and support to direct support staff on best practices, service delivery, and professional conduct.\nEnsure effective communication and collaboration between teams, managers, and service directors.\n\nIndividual Support & Rehabilitation\n\nAssess and address individual concerns, ensuring the involvement of appropriate support staff.\nDevelop individualized rehabilitation goals for workforce acclimation and personal skills development.\nConduct internal functional assessments and implement person-centered plans aligned with behavioral and therapeutic goals.\nLead Person-Centered Planning (PCP) and Interdisciplinary Team (IDT) meetings, maintaining updated audit schedules and compliance documentation.\n\nProgram Development & Community Engagement\n\nResearch, develop, and implement innovative program initiatives in collaboration with program teams.\nSupport community education and outreach efforts to increase program awareness and engagement.\n\nCompliance & Reporting\n\nEnsure adherence to Medicaid, Licensing, CARF, and other regulatory requirements.\nConduct file audits to maintain accurate service records and ensure compliance with Human Rights, Health & Safety, and state licensure regulations.\nPrepare and maintain quarterly and annual reports, ensuring timely data collection and outcome tracking.\nMonitor direct support professionals' (DSPs) compliance with documentation, procedural, and policy requirements.\n\nQualifications\n\nExperience & Skills\n\nBachelor’s degree or coursework in human services, psychology, behavior management, or related field preferred, or equivalent work experience.
