A company is looking for a Social Worker - Adult Inpatient. Key Responsibilities Evaluate complex patient and family situations with legal, ethical, and psychosocial components Develop and monitor individualized care plans, addressing barriers to goal achievement Participate in discharge planning for patients requiring post-discharge services Required Qualifications, Training, and Education Master's Degree in Social Work Required 2 years prior hospital-based social work experience Preferred 2 years prior hospital or community-based social work experience if supporting the HIV Clinic Preferred 2 years prior hospital or clinic-based pediatric experience if supporting American Family Children's Hospital (AFCH) Preferred Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW), or Licensed Clinical Social Worker (LCSW) in Wisconsin within 90 Days Required
