Lead with empathy during home visits, assessing patient and family needs and crafting care strategies that prioritize comfort, dignity, and peace. Empower families and caregiversthrough clear education on hospice services, symptom management, and available support resources. Guide and mentor your team, training Hospice Aides and LPNs to deliver safe, effective, and compassionate care at home. Collaborate seamlesslywith physicians, interdisciplinary teams, and facility staff to create smooth transitions and coordinated care plans. Provide hands-on expertisein pain control, medication administration, and symptom relief—bringing comfort when it matters most. Be a trusted resource after hours, participating in on-call rotations to ensure patients and families receive timely support on weekends and holidays. Maintain compliance and eligibilityby monitoring patient status and completing all certification and recertification documentation with precision.
