As a Homecare RN, you will deliver primary nursing care as ordered by the physician, develop individualized care plans in collaboration with the Population Health Case Manager, and ensure exceptional patient outcomes and satisfaction.This position offers a competitive hourly rate with an additional premium for flexibility. Flex nurses may also be eligible for assignment completion bonuses when meeting productivity and attendance criteria, with increased bonus opportunities during peak months.Coverage Regions:Jackson, Manchester, Grass LakeDearborn, Dearborn Heights, Garden City, Livonia, Taylor, Wayne, Westland, RedfordAnn Arbor, New Hudson, Plymouth, Canton, Ypsilanti, Novi, and WixomOakland County: Waterford, Pontiac, Bloomfield Hills, Clarkston, TroyWashtenaw County & Livingston County: Chelsea, Dexter, Brighton, Hartland, HowellWhat You Will DoConduct comprehensive patient assessments at admission and ongoing intervals.Develop and implement individualized, assessment-based care plans in partnership with physicians and case managers.Deliver therapeutic interventions and education to promote independence and improve health outcomes.Monitor and report changes in patient condition, adjusting care plans as needed.Utilize motivational interviewing and health coaching techniques to support patient goals.Document care accurately and timely in compliance with agency policies and regulatory standards.Mentor and support team members to ensure quality care delivery.
