Job Description Minimum Qualification: To have a minimum of bachelor's degree in a human service field A year's experience working with children and families, adolescents and adults withbehavioral health challenges CPR/First Aid Certification Knowledge of community resources Ability to work independently and on a team Have an excellent writing and communication skill Good time management and organizational skill Computer literacy Background checks required Reliable transportation and a valid driver's license and record Complete a 60hr on-the-job direct PRP supervision before working with minor andadolescent clients and 40hrs before working with adult clients.Job SummaryPsychiatric rehabilitation program (PRP) provides rehabilitative and support services that assistindividuals with a severe behavioral health challenges so that they could attain a higher level ofindependence. Services are individualized with a goal to assist and patient(s)/client(s) todevelop independent community living skills, including how to manage their behavioral healthchallenges while living in the community. Services may be provided on-site at a PRP facility oroff-site at an individual's residence, job or other location in the community. On-site PRP servicesprovide a structured environment where rehabilitation activities and services are providedpredominantly in a group setting. Individuals enrolled in a PRP program also receive: casemanagement services, including assistance with securing and maintaining entitlement;transportation to appointments; coordination of services and liaison with external services likesomatic, substance abuse and mental health practitioners.Rehabilitation coordinators are supervised by Rehabilitation Specialist, RehabAssociate and Program Director.Rehabilitation Coordinator Responsibilities Advocates for patient(s)/client(s) in the community Implement and assist the patient/client in meeting the identified rehabilitation planestablished in their Individualized Rehabilitation Plan (IRP) Analyze information from interviews, educational and medical records, consultation withother professionals, and diagnostic evaluations, in order to assess clients abilities,needs, and eligibility for services. Arrange for physical, mental, academic, vocational, and other evaluations to obtaininformation for assessing clients needs and developing rehabilitation plans. Collaborate with clients families to implement rehabilitation plans that includebehavioral, residential, social, and/or employment goals. Confer with clients to discuss their options and goals so that rehabilitation programs andplans for accessing needed services can be developed. Confer with physicians, psychologists, occupational therapists, and other professionals,in order to develop and implement client rehabilitation programs. Develop and maintain relationships with community referral sources such as schoolsand community groups. Monitor and record clients progress in order to ensure that goals and objectives are met. Prepare and maintain records and case files, including documentation such as clientspersonal and eligibility information, services provided, narratives of client contacts, andrelevant correspondence. Collaborate with community agencies to establish facilities and programs to assistpersons with disabilities. Locate barriers to client employment, such as inaccessible work sites, inflexibleschedules, and transportation problems, and work with clients to develop strategies forovercoming these barriers. Participate in job development and placement programs, contacting prospectiveemployers, placing clients in jobs, and evaluating the success of placements.
