The coordinator maintains documentation, supports care planning, and builds relationships within the Tribal community.\r\nEssential Duties\r\nLevel I -\r\nConduct home visits and welfare checks to evaluate and monitor Tribal Elders and their needs. Identifies economic, social, cultural, physical, and environmental factors that impact functioning.\r\nEvaluate the effectiveness of the care plans and recommend adjustments as needed.\r\nMaintain outreach and provide support services to Tribal Elders based on individual needs.\r\nConduct Activity of Daily Living Assessments to Determine eligibility for program services.\r\nConnect Tribal Elders with essential community resources, providing hands-on, in-person assistance as needed.\r\nPrepare and submit payment requests and Elder In-Home Care Timesheets for supervisor review and approval.\r\nAssist Tribal Elders with navigating resources such as Washington Connections, Apple Health and DSHS Long-Term Services and Supports; while collaborating with medical professionals and other service providers to support coordinated care.\r\nCoordinate transportation for health-related appointments as needed.\r\nMaintain transportation logs, reports, and related program documentation.\r\nPrepare purchase requests for program services, supplies, and activities for program manager review and approval.\r\nParticipate in Elder program activities and events in a respectful and supportive manner.\r\nMaintain accurate client records, case notes, and other required program documentation.\r\nParticipate in department meetings and trainings as appropriate.\r\nAssist Tribal Elders' physical and mental wellness, needs, preferences, and abilities to develop individualized care plans.\r\nWork with Tribal Elders, their support networks, and health care professionals to implement individualized care plans.\r\nDocument Trial Elders' progress, including charting referrals, home visits, and other notable interactions\r\nEvaluating Tribal Elders' progress periodically and make adjustments to care plans or services as needed to improve outcomes.\r\nLevel II -\r\nPerforms all duties of Level I, plus:\r\nAssist with program policy development as needed.\r\nUtilize Tribal Assister certification to assist Elders with enrollment in Apple Health and other eligible assistance programs.\r\nAdditional Duties\r\nAssist in identifying Tribal Elder needs to ensure optimal support and care.\r\nParticipate in program staff meetings, provider meetings, and workgroups as needed.\r\nAttend required trainings and continuing education necessary to effectively perform job duties.\r\nAssist with Title VI Elder Needs Assessment Survey and other surveys for information.\r\nThe Tribe values its commitment to community service and the well-being of its members. Knowledge of Native cultures, history, and traditions preferred.\r\nKnowledge of age-related health and social conditions affecting seniors preferred. Ability to maintain confidentiality and use tact and discretion.\r\nDemonstrate professionalism, strong communication skills, and sensitivity to the needs of Tribal Elders and their families.\r\nEducation & Experience Requirements:\r\nEducation Required\r\nLevel I -\r\nAssociate degree in human services, social work, healthcare administration, public health, nursing, or a closely related field preferred; or High School Diploma/GED with directly related experience in elder care, healthcare support, or human services is required.
