DescriptionJob SummaryThe Community-Based Day Support Professional assists individuals 1:1 with developmental and/or intellectual disabilities in accessing and participating in meaningful activities within the community. This role supports independence, social inclusion, skill development, and personal choice while ensuring safety and dignity in all settings.Key ResponsibilitiesSupport individuals during community outings, activities, and eventsAssist with daily living skills such as communication, social interaction, money management, and self-advocacyEncourage independence and participation based on individual support plansProvide supervision and ensure safety in public and community environmentsTransport individuals to and from community activities when requiredDocument daily activities, progress, and any incidents accurately and timelyMaintain professional communication with supervisors, team members, and familiesPromote inclusion, respect, and positive representation within the communityQualificationsHigh school diploma or equivalent (required)Experience working with individuals with disabilities preferredStrong interpersonal, communication, and organizational skillsAbility to work independently in community settingsCPR/First Aid certification or willingness to obtainValid driver's license with acceptable driving record (if transporting participants) Work Environment & ScheduleCommunity-based settings such as parks, libraries, job sites, recreation centers, and local businessesFlexible days and time, with schedules that may vary based on participant needs and activities
