HCB Waiver Case Manager

We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families by providing comprehensive case management services. This position involves assessing client needs, developing personalized care plans, and coordinating resources to ensure clients receive the support they require. The Case Manager will work collaboratively with various stakeholders, including healthcare providers, social services, and community organizations, to promote the well-being of clients. Responsibilities Conduct thorough assessments of client needs and develop individualized care plans. Provide ongoing support and guidance to clients through regular check-ins and follow-ups. Coordinate services across multiple agencies, including behavioral health, child welfare, and addiction counseling. Facilitate access to resources such as housing assistance (including Section 8), healthcare services, and educational programs. Mentor and supervise junior staff or interns in case management practices. Implement crisis management strategies to address urgent client needs effectively. Utilize motivational interviewing techniques to empower clients in their decision-making processes. Maintain accurate documentation of client interactions, progress notes, and service plans in compliance with regulatory standards.
