Registered Nurse - Home Health

Travel Registered Nurse (RN) Seeking an experienced Travel Registered Nurse (RN) to provide high-quality home health care services across multiple counties. This role involves delivering skilled nursing care, coordinating patient treatment plans, and working closely with physicians and interdisciplinary teams to ensure optimal patient outcomes in a home-based setting. Schedule & Work Details: Pay Rate: $51 to $53 per hour Schedule: Monday through Friday, 8:00 AM to 4:30 PM Weekly Hour Guarantee: 40 hours Coverage Area: Clinton, Washington, Marion, Bond, Madison, and St. Clair counties Mileage Reimbursement: Yes, between patient visits On Call/Weekend Requirement: 1 weekend per month and 1 weekday per week Key Responsibilities: Perform admission visits including completion of OASIS documentation Deliver skilled nursing care including wound care, catheter care, and PICC line management Conduct comprehensive patient assessments and monitor condition changes Initiate, implement, and maintain individualized plans of care in collaboration with physicians and care teams Communicate effectively with physicians and providers regarding patient status and care adjustments Provide patient and caregiver education to support health and recovery at home Maintain accurate and timely clinical documentation Participate in weekly case conferences and interdisciplinary team meetings Required Qualifications: Current Illinois (IL) Registered Nurse (RN) license Active CPR certification Previous home health experience Valid driver's license and reliable transportation Pride-Health offers eligible employee's comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance and hospital indemnity), 401(k)-retirement savings, life & disability insurance, an employee assistance program, legal support, auto, home insurance, pet insurance and employee discounts with preferred vendors.
