This role requires CHHA (Certified Home Health Aides) experience, a solid understanding of CHHA regulations in New York State, and comfort with OASIS (Outcome and Assessment Information Set) documentation.Job SpecificationMonitor and coordinate home health services, ensuring compliance with regulatory standards and quality care. Provide daily support to field staff, manage client care issues, and maintain accurate patient documentation in the agency's EMR system. Act as a liaison among clinicians, referral sources, patients, and caregivers, ensuring compliance with Medicare guidelines for seamless care delivery. Participate in case conferences, process referrals, manage medical supply orders, and follow up on entitlement verification issues.Candidate RequirementsAssociate's Degree in Nursing required; Bachelor's Degree in Nursing preferred. Current, valid RN license issued by the NYS Department of Education. Minimum of 2 years' experience in healthcare, with at least 1 year in home health.
