Our care management team is seeking a RN Care Manager to provide personalized care and support to caregivers and their loved ones. No night shifts or weekends required, and no on-call duties! As a RN Care Manager, you'll assess needs, create individualized care plans, conduct home visits, and ensure exceptional consumer care.\nWhat You Will Do::\nConduct home visits in assigned territory to assess home safety, gather changes in health status, and provide clinical coaching and support including review of medications\nFollowing an established care plan, provide ongoing coaching and support to caregivers via app, including resource referral, and reinforcement of best practices.\nDevelop trusting relationships with caregivers to influence change and reduce caregiver burnout\nDocument and review records for accuracy and timeliness, ensuring compliance with regulatory and accreditation standards\nPerform other duties as assigned\nWhat You Will Bring::\nMust hold an active Registered Nurse (RN) License in the state of employment\nMinimum two (2) years of recent experience in direct care of elders or adults with disabilities\nAbility to travel (usually by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required\nExperience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a health care setting\nProficiency in common software and web-based applications (e.g. MS Office, case management systems)\nAdditional state-specific qualifications may be required.\nJoin Our Award Winning Team:\nFounded in Boston, Careforth's caregiver programs and services improve health outcomes, keeping care at home longer.
