As a Medical Social Worker with Best Home Health Providers, you will shape how we care for patients in their homes, mentor a team of social workers, and help set the standard for compassionate, high-quality service across our organization.If you love the clinical side of social work and are ready to step into leadership—or grow the leadership skills you already have—we'd love to meet you.Please note that this role is per diem with a rate of $100 - $120 per visit.ResponsibilitiesLead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providersRecruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growthOversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standardsPartner with the Executive Director on the annual department budget and resource planningBuild strong knowledge of community resources and connect patients and referral sources to the right supportPlan in-service training and continuing education to keep the team's skills sharpContribute to quality improvement initiatives and help identify new ways to better serve our patientsPerform other duties as required for patient servicesRequirementsA Master of Social Work (MSW) from an accredited programAn active California license as an LCSWAt least 3 years of management experience, preferably in home careStrong knowledge of healthcare regulations, ACHC standards, and business operationsExcellent communication skills and the ability to build trust with patients, families, staff, and the communityA self-directed, organized, and flexible approach—you take initiative and work well with othersBenefitsFlexible ScheduleAbility to Convert to Full-Time Based On Performance If Interested Paid Sick DaysPaid Bereavement LeaveSkills Training & DevelopmentCompany Social EventsMileage Reimbursement for Patient to Patient VisitsReferral Bonus
