The Community Support Worker will facilitate individual mental health\nintervention and activities that are strengthâÂÂbased and focused on promoting recovery,\nsymptom reduction, while increasing coping skills and help to achieve the highest level of\nfunctioning in the community.\nEDUCATION& EXPERIENCE\n\nBachelor’s degree or at least 2 years of related work experience\nGENERAL RESPONSIBILITIES\n\nEnsure all necessary licenses and/or certifications remain current\nProvide medically necessary services to the consumer in accordance with the\ntreatment plan that is designed to meet the mental health, educational, vocational,\nresidential, financial, social and other needs of the consumer.\n\nFollow Mental Health Rehabilitation Services (MHRS) guidelines\nEnsure that treatment plan authorizations and reauthorizations, CALOCUS/LOCUS\nand updated diagnostic assessments and all other assessments are current and DO\nNOT expire\n\nServe as an intricate member of the SLS team to ensure continuity of care and\nsupports the services 24 hours a day and seven days per week, as necessary\n\nProvides structured faceâÂÂtoâÂÂface scheduled sessions to consumers at least once per\nmonth, or as needed based on the treatment plans\n\nProvide services in the home, school, office and community setting\nEnsure that the majority of mental health services are primarily rendered in the\ncommunity and limited services remain clinic-based.\n\nComplete documentation within 24 hours of service or report to supervisor via phone\nParticipate in mandatory training\nAssist with the facilitation of staff meetings and trainings\nCollaborate with and provide ongoing updates to the clinical team and psychiatrist,\nas needed\n\nIdentify, establish, and continually collaborate with community agencies, providers,\nand natural supports and advise team to make referrals as needed\n\nListen/respond to and interact with clients, coâÂÂworkers, management, and other\nprofessionals in a nonâÂÂjudgmental manner\n\nCommunicate objectively, thoroughly, accurately and concisely in a manner\nconsistent with the cognitive level of the persons involved\n\nDemonstrate an understanding and acceptance of the organizational mission and\nshared purpose to assist individuals by providing quality services\n\nParticipate in efforts to enhance service provision to consumers\nAct and interact to build cohesiveness\nAssist with building a positive rapport with other units and community agencies\n2\n\nDemonstrate professional work ethics, as evidenced by overall dependability,\nconscientiousness, and timeliness.\n\nOther duties as assigned\nPHYSICAL DEMANDS\nWhile performing the duties of this job, the employee is regularly required to perform\nphysical activities, drive, stand and walk. In addition, the employee may be required to\nrestrain a client or physically intervene to resolve an altercation. The physical demands\ndescribed must be met by an employee to successfully perform the essential functions\nof this job.
