Most tasks are carried out in the community, especially in family homes.\nDISTINGUISHING CHARACTERISTICS\nThis is an at-will direct service position within a program. The position is responsible for assisting and advocating for our members in all areas of treatment and coaching our members to reduce crisis incidents.\nESSENTIAL DUTIES AND RESPONSIBILITIES – (ILLUSTRATIVE ONLY)\nThe duties listed below are intended only as illustrations of the various types of work that could be performed. The omission of specific statements of duties does not exclude them from the position if the work is similar, related, or a logical assignment to this class.\nWorks in partnership with parents who are treated as experts concerning their families.\nHelps to conduct strength/needs assessment; assists with and participates in interdisciplinary team meetings.\nCompletes all tasks associated with both fidelity to the START Model, program certification of the START Model, and individual certification in the START Model.\nResponsible for submitting/entering outcomes/incident data into the identified database or providing this to the person responsible for entering the information.\nCoordinates care as needed, internally and with outside agencies; Coordinates care with system partners.\nWith the direct input from the child and family, helps to enlist the involvement of formal and informal community resources to support the family; sees the family weekly for periods of time/days that may vary depending on the needs of the family.\nKeeps START Coordinator and supervisors informed of significant treatment or care coordination issues.\nSees the family weekly for periods of time/days that may vary depending on\nthe needs of the family.\nDischarge planning/transitional service development; attends to the safety, health, and well-being of clients; responsible for all required record keeping as indicated by funder, certifying agency, and TPCP.\nSafely transports clients and other passengers to and from appointments and activities.\nProvides prompt intervention in the event of a crisis, both to stop the crisis and to notify when indicated, persons and agencies necessary for the resolution of the crisis; provides “on-the-spot” counseling that is both helpful to the clients and consistent with the philosophy of the program.\nIs personally responsible and held accountable for work hours and time management as coordinated with the Team Leader.\nProvides support to other staff members and program members as needed.\nProvide after-hours, “on-call” coverage to program clients on a rotational basis.\nMeets and maintains service productivity standards as set by the program; completes paperwork as assigned promptly.\nLICENSES; CERTIFICATES; SPECIAL REQUIREMENTS:\nCalifornia driver’s license & current vehicle insurance/registration.\nReliable means of transportation capable of passing vehicle safety inspection if more than five years old, excluding all modes of two-wheeled transport, inclusive of bicycles, mopeds, and motorcycles.\nBachelor's Degree and 4 years of relevant experience or,\nMaster's Degree and 2 years of relevant experience.\nSchedule: Monday – Friday, 8:30 am - 5:00 pm with the flexibility to work from 7:00 am - 7:00 pm.\nCompensation: $24.00 - $26.00 an hour.\nInterested?
