All our services are trauma-sensitive and client-centered and are provided at no cost to our clients. All programs are low barrier.\n\nRP is led by survivors with a focus on reflection, innovation, and connection. We believe in fostering relationships, both in our direct service work and throughout our internal work culture. You are flexible, creative, and take initiative in handling emergencies.\n\nWhat qualifications are required for this position?\n\nYou also have:\n\nA Bachelor’s degree in Social Work, Human Services, Psychology, or related field preferred.\nMinimum 2 years of experience in case management, housing navigation, or homeless services.\nExperience working with families and individuals experiencing housing instability.\nKnowledge of Housing First principles and local housing resources.\nStrong written and verbal communication skills.\nA valid CA driver's license, proof of insurance, and be approved by RP insurance as a driver in order to drive agency vehicles. If a staff member cannot be approved by RP insurance as a driver, employment might not be able to continue. Instances where staff cannot be approved to drive will be reviewed on a case by case basis to determine if employment can continue in their role without driving agency vehicles. Items considered include but are not limited to: the shift the staff member holds and the frequency/necessity of driving during that shift, the amount of time before the staff member may be approved to drive by the insurance carrier, the availability of other staff on site to drive agency vehicles as program needs require, etc.\nAbility to complete all required background checks. Additional requirements may be deemed necessary, including fingerprinting (Live Scan), completion of all Community Care Licensing (CCL) documents, a suitability process through ORR and ability to meet CCL licensing requirements.\n\nWhat You Will Do\n\nThe Case Manager for the ESH Program provides comprehensive, on-site case management services to families and single adults experiencing housing instability. This role is responsible for developing individualized case plans focused on stability, resource connection, and permanent housing placement.\n\nThe Case Manager works collaboratively with participants to identify barriers to housing, establish realistic goals, and coordinate supportive services that promote long-term self-sufficiency. This is an on-site position requiring consistent in-person engagement to ensure effective service delivery, relationship-building, and accountability.\n\nResponsibilities include\n\nIndividualized Case Management & Housing Stabilization\n\nConduct intake assessments and develop individualized, goal-oriented case plans.\nCollaborate with participants to identify housing barriers and stability needs.\nFacilitate housing navigation, including rental applications, landlord engagement, and housing search support.\nSupport participants in obtaining necessary documentation (ID, income verification, benefits).\nProvide referrals and warm handoffs to community-based services (employment, healthcare, behavioral health, childcare, etc.).\nMonitor participant progress toward housing and stability goals.\n\nDocumentation & Compliance\n\nMaintain accurate, timely, and thorough case documentation in agency systems.\nEnsure compliance with City of Alameda ESH contract requirements.\nComplete required reports, case notes, and service tracking documentation.\nAdhere to confidentiality standards and mandated reporting laws.\nParticipate in audits and monitoring visits as required.\n\nOn-Site Engagement & Participant Support\n\nMaintain consistent on-site presence during scheduled hours.\nConduct regular in-person check-ins with participants.\nSupport crisis intervention and de-escalation when needed.\nCollaborate with on-site staff to ensure coordinated care.\nPromote a respectful, safe, and supportive program environment.\n\nCollaboration & Community Partnership\n\nCoordinate with City of Alameda representatives as needed.\nParticipate in case conferencing and multidisciplinary meetings.\nBuild and maintain relationships with local service providers.\nSupport cross-department collaboration within the agency.\nComplete functional timesheets by assigned due dates in compliance with time allocations.\nActively participate in reflective supervision sessions.\nCreate and maintain documentation of processes and procedures related to job duties.\nOther duties as assigned.\n\nWorking Conditions\n\nThis is a full-time, on-site position. You are expected to be available on-site as necessary given business needs. You must be able to stand for long periods of time and perform physical activities, including lifting up to 25 pounds.
