the emergency department - Primary Care offices - inpatient wards or other Medical Center or community settingsThe incumbent will function with very little supervisory controlThe incumbent must demonstrate significantly advanced clinical skills and be able to function independentlyThe incumbent incorporates complex multiple causations in treatment of Veteran patients - including making psychosocial assessments - providing case management and providing psychotherapy within approved scope of practiceMajor duties include - but are not limited to: PC-MHI Responsibilities Serves as Team Member for an inter-professional clinical team: Primary Care-Mental Health Integration (PC-MHI) - co-located with the Primary Care medical teams - with an emphasis on integration of mental health services within primary careAs a full clinical member of the PC-MHI Team - provides a full range of diagnostic and evaluation services appropriate to the primary care setting - especially in the areas of general psychological evaluation of emotional and behavioral problems and focused cognitive assessmentsProvides a full range of psychological interventions consistent with training and expertise - with special emphasis on brief and evidenced-based interventions found to be effective in primary care settingsFacilitates patient and family education appropriate to the primary care setting and the specific presenting mental or behavioral health issuesProvides consultation services for primary care providers to maximize the effectiveness of their interventions in reducing health-risk behaviors and enhancing health-promoting behaviors - with a significant emphasis on a prevention-oriented model of careSupports PCP decision-making - building PCP interventions and improving PCP-patient working relationshipsProvides clinical care management when appropriate directly to an assigned cohort of patientsProvides the expertise and leadership so that the integrated care and collaborative practice model is fully implemented -
