Ensures daily encounter notes are properly completed and maintained.\nParticipates in Case meetings and attends other meetings regarding client care and as required by state/fiduciary regulations.\nProactively determines client care needs and ensures proper care is being provided.\nPerforms Community Support duties if and when needed, as outlined in the client’s treatment plan.\nSupervisory Responsibility\nThis position manages all employees under the case managers scope, which may include.\no Responsible for performance management of direct care staff.\no Ensure all CTL policies and procedures are being enforced.\no Coaching and Development.\no Training and Education of Direct Care Staff.\no Create and administer weekly schedules and ensures shifts are properly covered on a daily basis.\nCompetencies\nCommunication.\nConsultation.\nCritical Evaluation.\nCultural Awareness.\nLeadership & Navigation.\nRelationship Management.\nEthical Practice.\nWork Environment\nThis job operates in the community, specifically in client’s homes, 2-3 days per week, in the office 1-2 times per week and provides a work from home flex day 1-2 times per month. (Due to Covid-19 restrictions, in home visits are based solely on the client’s needs). This role uses standard office equipment 1-2 times per week.\nTravel\nTravel is required throughout the region. A company vehicle will be provided along with training from the Worker’s Compensation Trust.\nPhysical Demands\nWhile performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; talk and hear. The employee must occasionally lift or move office products and supplies, up to 20 pounds.\nPosition Type/Expected Hours of Work\nThis is a full-time position. Days and hours of work are Monday through Friday, 8 a.m. to 4 p.m. (hours can be flexed).
