The Program Coordinator provides case management and supportive counseling to the clients within two locations; advocates for the clients’ needs and supports them to meet the goals on their Individualized Service Plans. The Program Coordinator also assumes responsibility for the facilities and for supervision and oversight of the House Managers.\n\nEssential Job Functions\nUsing Motivational Interviewing and a collaborative approach, conduct in-depth assessments to best determine the immediate client concerns including, but not limited to: financial, legal, mental health, substance abuse, employment, health care and housekeeping;\nProvide comprehensive case management, meeting at least once a week with each resident; Works with residents to Develops individual service plans (ISP) which include educational, vocational, medical, life skills goals. Meet with residents in the program weekly to review ISP progress and plan next steps\nAvailable to the residents for counseling and crisis intervention, and makes referrals to community programs for follow-up and treatment for substance abuse mental health, income enhancement and housing\nObtain resources and services as requested and/or needed to address barriers to independent living including but not limited to; CORI mitigation, credit repair, and assistance with financial applications\nDevelop partnerships and collaborative relationships with internal and external service providers to ensure client receives comprehensive services;\nAssist client with move-in transition supports including but not limited to; furniture and furnishings assistance, budget support, and other resources and services as needed;\nDocument all interactions with clients utilizing appropriate reporting mechanisms, including web-based data collection systems;\nResponsible for coordinating the houses programs and services;\nProvides direct supervision to house managers and ensure proper coverage of all shifts;\nAttends a weekly meeting with the Program Manager and trainings as required;\nServe as liaison with Community based organizations, attends meetings with such organizations as needed;\nFacilitates residents’ house meeting, arranges for guest speakers, and advocates for the residents at staff meetings;\nCompletes reports for program as needed;\nAttends meetings with outside provider agencies;\nPerforms other tasks as requested by the Program Manager, Director of Individual Services or the Sr. Director of Programs\nObserves all shelter and agency rules and policies.\n\nEEO STATEMENT\nHeading Home, Inc. is an Equal Employment Opportunity (EEO) employer and welcomes all qualified applicants.
