Employment TypeFull timeShiftDescription:Clinical Hospice Coordinator –Covering referrals for Trinity Health Oakland Hospital-PontiacFacilitates hospital-to-home transitions by coordinating referrals to hospice services, ensuring timely, effective patient placement. Serves as a key liaison between hospital staff, physicians, patients, and families—providing education on services, supporting discharge planning, and ensuring a smooth continuum of care.
