Team Collaboration:\nclosely with the Dsignated Coordinator and care team to ensure smooth service delivery and compliance with regulations. Please Note: The duties\nd responsibiities of this role may change in the near future due to program adjustments. While we are unable to predict exactly how the role will evolve, we are committed to ensuring transparency and support during any transitions. Candidates should be flexible and adaptable to potential changes in job responsibilities. Key Responsibilities (Cur\nnt): Provide direct care and s\n\nt to individuals living independently, helping them acquire essential adaptive skills. Assist individuals in develop\npersonalized 24-Hour Emergency Back-Up Plans. Support the Designated Coordina\nwith clerical tasks, ensuring efficient service delivery. Document progress, report inciden\nand maintain compliance with 245D regulatory guidelines. Collaborate with a wide range of pr\nssionals, including waivered service providers, mental health teams, and social workers. Participate in an on-call rotation fo\n4-Hour Emergency Assistance services. Comprehensive Benefits Package: 401(k\nith company match up to 3.5% (e\n\ne after 1 year). Health, ental, and vision insurance with emp\nr-paid contributions (eligible after60 days). Hold at minimum a 2-year degree in Human Services or a related field, AND at minimum\nears of direct experience working with individuals with Intellectual/Developmental Disabilities or related conditions Working Conditions: The Unit Based Services Program Assistant works in various communi\nsettings, frequentl\ntraveling to clients' homes.
