PHYSICAL THERAPIST - PT/PRNLOCATIONS: NORTH RALEIGH, WAKE FOREST, FRANKLINTON & LOUISBURGHome Care Agency is looking for a Physical Therapist to join our home care agency.As a Physical Therapist, you are responsible for initial assessment, reassessment, developing care plans and setting measurable goals for clients. You are charged with carrying out clients care as established in the Agency's policies and procedures.Responsibilities:Perform initial assessment and ongoing evaluation and treatmentDevelop and implement an individualized plan of care with measurable goals for patientsMonitor patients' response to treatmentConduct individual patient therapy regimensSupervise Physical Therapy AssistantsWork to identify and solve clients' concernsComplete appropriate documentation according to agency's policies and proceduresTeaches, supervises, and counsels the client and family in the physical therapy program.Investigates and resolves patient/family member concerns regarding patient care.Ensures the highest quality of care is provided to patients by reviewing documentations.
