Patient Care Coordinator

This is a great opportunity to join a dynamic, fast-growing company committed to making the world a healthier place for all.We are currently seeking a Patient Care Coordinator to act as a liaison between our provider and patients to monitor treatment plans, connect patients with healthcare providers, and evaluate their progress. To be successful in this role, the Patient Care Coordinator must be responsible for consulting with patients and working with providers to coordinate interventions.The ideal candidate must be knowledgeable about healthcare practices and have the ability to delegate scheduling tasks and assure execution.Summary of Responsibilities:Build rapport and establish trust with the patients, providers and Scheduler(s)Conduct weekly meetings with assigned provider(s) to discuss active patient list; assure changes/modifications are implemented to accelerate care and get better outcomesAssure patient compliance by reviewing the daily schedule ongoing; delegate changes/modifications to the Scheduler(s); follow- up to assure completionCarryout coordination for patient imaging as requiredCollaborate with Medical Team for MD appointments as requiredProvide advanced knowledge of Alliance Ortho services and technologyComplete mastery of role and responsibilities of a Scheduler Requirements: Great verbal and written communication skillsStrong analytical thinking and ability to be solutions-orientedExcellent customer service skillsCompassion and empathyAdditional skills/abilities (preferred not required): BA/BS in Healthcare Administration, or related field preferred, but not requiredPrevious experience in a similar practice: Sports Medicine, Chiropractic, Physical Therapy, Pain Management, Orthopedic/Spine Surgeon.Work Location: One locationJob Type: Full-time
